Nurse Prescriber, to £35K plus car or car allowance
Rate: £ 30 to £ 35 K - per annum + car / car allowance

•To work as a member of a multi-disciplinary team to provide effective and high quality care to patients with respiratory and chronic diseases in a home environment.
•To act as a care coordinator by taking responsibility for the overall assessment, care planning and review of a caseload of patients requiring specialist nursing.
•Assess and evaluate specialist clinical care using a range of assessment tools, clinical guidelines and approaches (e.g. Pain, sleep, nutritional status, speciality specific tools).
•Ensure that patient care plans are regularly updated, changes implemented and that patients’ records and documentation are maintained and current.
•Organise meetings to discuss patient management as required.
•Participate in the development of evidence – based patient care policies and procedures. Develop and contribute to clinical guidelines and protocols where appropriate and ensure they are implemented.
•Provide specialist support to the clinical team in dealing with the complex needs of different client groups and when imparting or receiving sensitive, complex or contentious information.
Assessing and talking to patients about their problems and discussing the best way to plan and deliver their care;
•To follow clinical risk assessment and management procedures in accordance with the Company policy.
•To promote the involvement of users and family members in all aspects in the planning and delivery of care
•Maintain accurate data in accordance with NMC and the Company Policy for ensure evaluation and audit purposes.
•Ensuring the ongoing good communication and effective relationships with relatives and fellow professionals in the team and attending regular meetings to review and monitor patients' care plans
•Assisting in planning a smooth transition of patient care from the hospital to the home.
The following qualifications are essential:
•RGN part 1 NMC Register
•Post registration qualification in specialist area (e.g. Diploma in Chronic Disease/Asthma or COPD)
•Nurse Prescriber
