To work as a member of a multi-disciplinary team to provide effective and high quality care to patients with respiratory and chronic diseases in a home environment.

To act as a care coordinator by taking responsibility for the overall assessment, care planning and review of a caseload of patients requiring specialist nursing.

Assess and evaluate specialist clinical care using a range of assessment tools, clinical guidelines and approaches

Ensure that patient care plans are regularly updated, changes implemented and that patients’ records and documentation are maintained and current.

Participate in the development of evidence based patient care policies and procedures.
Provide specialist support to the clinical team in dealing with the complex needs of different client groups and when imparting or receiving sensitive, complex or contentious information.

Assessing and talking to patients about their problems and discussing the best way to plan and deliver their care;

Assisting in planning a smooth transition of patient care from the hospital to the home.

The following qualifications are essential:

RGN part 1 NMC Register
Post registration qualification in specialist area (e.g. Diploma in Chronic Disease/Asthma or COPD)
