Rapid Response Nurse to £35K
Rate: £ 26 to £ 35 K
To contribute to the growth and success of the Company, by assisting in the development of a high quality management home care service by working within a multi-disciplinary team to prevent hospital admissions, to promote early discharges and to provide care and rehabilitation to maximise independent living.
The team shall also provide a rapid response service, utilising hospital, private and community resources to prevent admissions. The nursing element of the team provides care to acute medical and surgical patients through the use of protocols and management plans. Working with agreed management care plans and protocols. Liaise regularly and appropriately with the virtual ward recognizing that all patients under the care of the organisation would otherwise be in an acute hospital environment.
Optimise bed utilisation and reduce patient length of stay by promoting timely, safe discharges of patients to the most appropriate and agreed environment.
Support the Medic and virtual ward at all times and assist in the day-to-day smooth running of the service.
Work in close partnership with team members, hospital colleagues and external agencies such as Social services; Primary Care Trust (PCT); Housing Associations; GPs and voluntary organisations.
Independently prescribe medication for conditions within own field of expertise
Being flexible with regard to working hours including on-call coverage and scheduled weekend and evening work.
Experience & Qualifications:
Advanced clinical practice skills.
Significant years post registration experience.
Recent primary and community nursing experience.
Nurse prescribing.
Registered first level nurse MSc or equivalent
Relevant nursing/health degree
A current clean driving licence is essential as the role involves travel across the region.
