The role:

-To work as a member of a multi-disciplinary team to provide an effective and high quality mental health service to patients that meets best practice criteria.
-To act as a care coordinator by taking responsibility for the overall assessment, care planning and review of individual service user needs.
-Caring for patients who are experiencing acute mental distress or have an enduring mental health problem;
-Assessing and talking to patients about their problems and discussing the best way to plan and deliver their care;
-Building relationships with patients to encourage trust.
-Ensuring the correct administration of medication, including injections, and monitoring the results of treatment;
-Responding to distressed patients in a non-threatening manner and attempting to understand the source of distress;
-Preparing and participating in group and/or one-to-one therapy sessions.
-Preparing and maintaining patient records;
-Producing care plans and risk assessments for individual patients;
-Maintain accurate data in accordance with NMC and the Company Policy for ensure evaluation and audit purposes.
-To ensure that legal requirements of the Mental Health Act 1983 and the NHS and Community Care Act 1990 are met. Incorporate all changes in legislation.
-The role holder will have experience and knowledge in the following:
-Suitable post registration experience in a busy working environment
-Understanding and application of Risk Management

You will be a Registered Mental Health Nurse (RMN / RNMH)

A current clean driving licence is essential as the role involves travel across the region.
